Atrial fibrillation (AF) is the most prevalent cardiac arrhythmia with an estimated 33.5 million patients globally in 2010.

Goal

Reduce all-cause hospitalisation and mortality in patients with AF. To test this hypothesis, the iCARE-AF clinic will be compared with standard care in a prospective, multi-centre, controlled trial.

Aims

To implement and disseminate integrated outpatient services (AF-clinics).
Improve efficiency and outcomes of AF treatment by applying integrated care approach.
To investigate the role of nurses and allied professionals in this integrated care approach.
To contribute to healthcare reform and redesigning AF practice.
